I hereby that this Photography Release/ Consent Form (hereinafter referred to as “Form”) becoming affective on the said date as signed.

I hereby authorize the photographer to take, make use of all photographs of me to be use for promotional materials.

Upon the usage of my photographs, I consent to such materials becoming the property of the PG Creative Media and that I will no longer be entitled to them, provided that it is done for lawful purposes.

I authorize the use of my photographs for, but not limited to, publication on the internet, social media, magazines, journals, books or prints provided that it is done for lawful purposes.

I hereby acknowledge that I will not be entitled to payment or any sort of charge or compensation for the use of the materials

I hereby release all rights to any, but not limited to claim, rights, demands, and/or any cause of action by me or my representatives or anyone else.

Furthermore, I hereby waive my right to any royalty or any other compensation with regards to the usage of the photos referred to in this form.

I am over 18 years of age or that my Parent / Guardian has read this and understands its meaning.
